Competition in my case comes from insurance companies and general practitioners since patients have little control. Efficiency is rewarded with more doctors and insurance companies preferring your hospital and thus sending more non-emergency patients the way of a hospital/facility.

I don't know what your preferences are for health care, so I can't answer that one.
