limited to an elevation of 200'.

The flight was descending to land and the helicopter was high (although I read only 100' high?) so there's one hole in the swiss cheese model. Another factor could have been with all the urban lights and aircraft in the corridor, the helicopter pilot may have misidentified the aircraft when it accepted visual separation from ATC so it simply didn't expect the jet to be there.

The view you have from the ground, which someone posted a video of earlier, is not the view you have from the helicopter's pilot vantage point.
