probably about 5-6 errors that occurred here, and just very bad luck as well.

There have been 30 near misses near there in the last 40 years. A near miss basically has all the same issues, but just that no collision actually occurred. Like most things in life, near misses get dismissed mostly until something tragic happens that gets the right fixes in place.
