shooter was known to be mentally ill (and in this case, had "serious emotional issues that required a doctor's attention")... the shooter's family was also reportedly convinced he/she should not own a weapon.

None of those people took a step to ensure his/her condition was reported and a determination made about his/her ability to purchase or possess a firearm.
