There is actually a lack of regulation in some states (most states) The provider is forced to participate.
The insurance companies pay what ever they want and some do not pay at all especially true with Medicaid and Medicare. We have had to stop taking insurance from certain companies. United healthcare is the worst one that I know of. Blue cross and Blue shield is ok but not Florida Blue
