Huge gains in productivity and GDP. Some of that led to things like food on the run, junk food, sedentary lifestyle and so the average American is less healthy than they used to be, despite on a macro level things like smoking no longer as widespread, etc.

The health insurance markets did not shift with the increase in unhealthy people. Over half of the healthcare in this country is consumed by 5% of the population. Medicaid and Medicare were early attempts to patch who the profit oriented healthcare companies no longer wanted to insure. The old and the poor.
