possibilities. Either it is an individual here and there that is messing up, or it is systemic.

If it is a systemic problem, the ones responsible would be in upper management. It seems to me that the Mayor and those making the decisions on hiring, firing, setting teh standards and so on would be where the buck stops for systemic problems if there are any.

Why blame the workers for bad decisions of those above them?
