here's what I see

And this is just my outsider suspicions. I have no industry understanding.
1. We have become a very unhealthy society. Our collective health is breaking the system.
2. People are getting rich off of healthcare. In particular, medical device companies, insurance agents, and pharmas.
3. We have no market forces to drive prices down. See lasik which is not covered by most plans. Elective medical procedures are affordable.
4. Our food industry gets a lot of blame for our society becoming unhealthy. We have a heavy corn-based diet. Obesity is an epidemic.
First order of business is to create premium price controls. I know this sounds horribly socialistic, but we all need healthcare and because out of control pricing has been thrown on us, we don't really have a choice. My family is a year or two away from being uninsured and we are considered high income earners.
Let the healthcare industry figure out how to make money off of the the roughly two trillion dollars it now takes in. I know it might mean chicken instead of steak at the Christmas party, but I bet they can do it.
Bottom line is they charge so much because they can. There are no forces holding costs in check. That's where to start.
