He doesn't have a grasp of the issues because he's too lazy to read up on them. That's what a lot of the job of the POTUS is. You have to understand the issues by reading up on them. It's not even that hard. You have binders of issue summaries prepared for you all the time, on everything.

The idea is, like a CEO, you can't make executive decisions if you don't understand the issues.

Trump is vigorous for a 70 year old. Being a billionaire will preserve your body. But he's lazy. Intellectually and physically.
