what they did to deserve it? He's suggesting healthy people are good and unhealthy people did it to themselves.

Certainly there are cases where unhealthy lifestyles lead to health problems, but he doesn't make that distinction. And even if you do, who's going to decide that you created your own health problems, so you pay more?
