One theory is the tech screwed up and didn't have him take off the chain around his neck...hospital's fault.

One theory is he walked into an unathorized area while the machine was running. If that is the case then it should have been locked down...hospital's fault

Many hospitals use metal detector wands on each patient because they don't trust they don't have any metal on them...sounds like this one didn't or didn't use it.

Either way I would say the hospital is on the hook.
