Males and females have different characteristics and tend to behave differently. Sure those behavior patterns may take the shape of a Bell Curve and some males may behave more towards the female end of the spectrum on given behaviors than most, and vice versa, but they every cell in their body shows what their true sex is.
Society may have "constructs" regarding how boys and girls/men and women behave and some people may rebel against those constructs.
But the break with reality, the psychosis, is when a boy thinks he is a girl, or when a woman thinks that she is a man. When they take hormonal treatments to prevent the healthy biological development of their bodies, or surgeries to make them look more like or behave more like the opposite sex they are deluding themselves in thinking that they have become a different sex.
Trying to force their delusions upon society is pathological.
