You could possibly get to the bloodstream through the glomerulus of the kidney - assuming you had severe enough diabetic nephropathy and your string was about the size of albumen - but you can't get to the GI tract from there safely. I think it might be possible to get to the bile duct from the liver - but I think there's too much liver from the portal vein to the lobe that connects to the bile duct.

Easiest answer is the length to go from your urethra just proximal to the prostate, transect the urethra and tunnel through the gooch and then transect the rectum to get out of the anus. A good 8-12 inches should do it.
