title that has earned it. Especially as a teacher or a position of authority because Dr adds to their perceived (and real) expertise.
Insisting on being called Dr outside of your profession, that is bizarre behavior. Even for medical doctors or DOs.
As for lawyers, they don't need to go by Dr to be a douche. They just add esquire to the end of their title and that's infinitely more douchey than Dr.
