e to me for a piece of paper that had gotten damp/wet or had been crumpled a bit.

The vertical lines on each side don't look real if they were supposed to originally be straight. But I think they were never straight on any original, assuming there was one.

I would be surprised, however, if any organization as sophisticated as that paper makes it look would actually hand out instructions like that to a bunch of people. It would be pretty likely to get into the wrong hands from their perspective.
