This would be like FICA, with holding like everyone pays for defense, we all would pay in for healthcare. No mandate you sign up for you - it is deducted from every check. As for the Insurance industry it could well adjust to cover niche services - boutique speciality insurance. Wouldn't be like it used to be b/c the way it used to be (the way it is now) fails.
