I filled out paperwork, spelled correctly.
She entered incorrectly.
She turned the computer screen so I could verify. I told her that it was spelled incorrectly, she asked how it was spelled. I spelled the name twice, verbally, correctly. The sheet, too, was in front of her, with the name spelled correctly.
She typed what I said, while apologizing, and finished the process. I didnt re-check her work. She never turned the screen again for me to re-verify. She had two opportunities, and I thought that someone couldnt possibly **** it up twice.
A month goes by and I receive my title with the TOD spelled incorrectly.
